How to Access CareUUCLV currently offers visits with trained Pastoral Care Associates, home-cooked meals delivered to homes, and occasional rides to and from church or other locations for members and friends of the congregation.
If you would like to meet with a Pastoral Care Associate for accompaniment through a difficult situation, or if you would like to arrange for transportation, meals, and/or similar from a Care Task Team member, please email our Logistics Team at [email protected]. One member of the team will be available to assess and forward your request to the appropriate Pastoral Care Associate or Care Task Team Member. That person will get back to you within 24 hours. The minister will also be alerted to your request. For emergency situations call 911. Emergency Contact FormsThe Pastoral Care Associates make available Emergency Contact Forms for all congregants to complete. When you have completed filling out your form, please return it/them only to a Pastoral Care Associate member or Reverend Rachel. The completed forms will be kept locked up in a secure location and the information on them will be kept strictly confidential. Letting the Minister have access to these forms is important so that she can contact the appropriate people in case of an emergency, especially if you live alone. Some safety professionals have suggested that the emergency contact be someone(s) who does not live with you. Thank you in advance for your participation in this critical project. Forms are available in the church office.
